   Ways to Pay:
   

   1. Pay the $50 deposit (or full payment) on-line at How's Bentley E-Store via the left navigation bar.
   2. write a personal check and postal mail it to the address on the form.

2. Deliver the forms with deposit or on-line receipt to How's Bentley's address which is listed on the agreement form. People who pay on-line may fax the forms along with the on-line receipt to the fax number listed on the agreement form.
3. After receiving the forms and deposit, How's Bentley will contact you to schedule your services.

Problem Behavior Services

Problem behavior services require two or three forms, depending on the nature of the problem(s). Please see the options below.

1. People who desire behavioral services for NON-AGGRESSIVE behaviors should complete these two forms.
